Filled application form, address proof, proof of date of birth, police report for the lost or damaged passport, self-attested copy of the first and last pages of the passport with ECR/Non-ECR page, and affidavit showing how the passport got lost or damaged as per Annexure F.

First, you need to report to your nearest police station for the lost or damaged indian passport and obtain an FIR., You need to gather all the documents to apply for a New passport online through the Passport Seva Kendra and fill out the application and upload all the documents, and pay fees for the Indian passport application. After paying the fee, book an appointment for confirmation of your identity at the Passport Seva Kendra, and wait for the processing of your new Indian passport.

    You can apply for the re-issue of your passport at any nearest Passport Seva Kendra (PSK) or Indian Mission (If abroad) and submit all the required documents along with your passport application form. You will need to submit the details of your previous passport, including your passport number, date of issue, place of issue, and date of expiry. If you don't have this information, you can contact the respective mission or post of your passport-issuing country.

    Firstly, do not panic and immediately report the loss or damage to your nearest police station and get a copy of the FIR. After that, report the loss online to the Passport Seva Kendra and apply for re-issuance of your passport. Then, collect all the required documents for your application and pay the applicable fees. Finally, book an appointment at PSK for document verification and submit your biometric data. Then, visit PSK on the scheduled date. Once all this is done, track the status of your application. The process isn't complex if you follow the correct steps.
