How to Add Surname in Indian Passport?

Follow the steps given below to change your surname in your Indian passport online.  

  1. Go to the official Passport Seva portal. 
  2. Register or log in to the portal. 
  3. Now, click on the "Apply for a Fresh passport/Re-Issue of passport"
  4. Choose alternative 1 for the online application. 
  5. Select 'Re-issue of the Passport' and enter all the details you need to change. 
  6. Enter all the required details in the online application form. 
  7. Agree to all the terms and conditions, and click on the continue button. 
  8. Select documents as proof of address and birth. 
  9. Now, you need to print the online application and pay the required fees. 
  10. Book an appointment with the nearest application center. 
  11. Upload all the supporting documents and download the final receipt. 
  12. After completing all the steps, you need to visit the appointment center and submit the final receipt along with the necessary documents, and you will get your updated Indian passport at your registered home address.

      Gather the documents given below to change your surname on your Indian passport. 

       

      For name change after marriage: 

      • Copy of the married woman's passport. 
      • Copy of the husband's passport. 
      • Self-attested copies of the marriage certificate. 

       

      For name change after divorce: 

      • Copy of the divorce certificate. 
      • Affidavit or the non-judicial stamp paper.

       

      For changing name after remarriage: 

      • You need to submit proof of divorce. 
      • If the death of husband has died, then you need to provide the death certificate of the husband. 
      • Copy of the husband's passport. 
      • Self-attested copy of the marriage certificate.
