Yes, OCI cardholders can change their genders, but there is one condition: for the gender change, you must have a new passport that shows the gender change before applying for the online OCI miscellaneous services. After updating the new passports with a gender change with a medical certificate, which is issued by the foreign authority, it should be apostilled by the Indian mission abroad. Then an OCI cardholder can easily change his or her gender on the OCI card.
I have applied for the gender change in my OCI card by submitting a medical certificate and a new passport with the new gender, then I have filled out a miscellaneous online application for the gender change, and after uploading all the documents, I submitted my online application, and got it in just a few weeks with the updated gender on my OCI card.
